Job summary

The City of Miramar is seeking a Senior Wellness Facilitator to design and manage wellness, health and fitness programs for seniors in the community. You will develop a caregiver education program to support families caring for elders and help reduce health disparities in medically under-served populations.

You will work with various community providers, conduct assessments, maintain records, and ensure compliance with regulations while supporting elders to age in place.

Qualifications

• Bachelor’s degree in Rehabilitation, Health and Human Services, Public Health, Education, or related field.
• Minimum of three years’ experience with a geriatric population.
• Proficiency in Microsoft Word, Excel, and PowerPoint.
• Certified in First Aid and CPR.
• Possess and maintain a valid State of Florida driver’s license.
• Subject to Level II background screening and health physical.

Responsibilities

• Plans, develops, and implements health, wellness, and fitness programs for elders.
• Coordinates with community providers and performs intakes to determine service eligibility.
• Develops medication management and education programs for seniors.
• Creates comprehensive fitness programs to meet diverse needs.
• Provides referrals and assists in obtaining health and fitness services.
• Maintains confidential client records and grant reporting as needed.
• Works varied shifts including days, evenings, nights and weekends.
