Job summary

EmpowerMe Wellness is seeking a Care Coordinator to provide administrative oversight in senior living communities, optimizing therapists’ schedules and helping to build caseload while supporting care coordination touchpoints.

You will handle intakes, coordinate scheduling, conduct wellness trainings, and maintain strong relationships with residents, families, and staff, ensuring consistent, high‑quality care across the organization.

Qualifications

• Certification in healthcare, business administration, or related field.
• Two years of relevant experience in a senior living setting preferred.

Responsibilities

• Identify seniors’ health needs and help build caseload; communicate needs to site leadership; maintain on-site presence in communities.
• Support intake process for new residents, scheduling evaluations, and obtaining documents from providers.
• Provide care coordination through touchpoint visits; conduct wellness classes and screenings under licensed supervision.
• Assist in coordinating therapists’ weekly schedules; support onboarding and interim coverage for vacancies.
• Review operational reports and assist with weekly billing closeouts.
