Qualifications

Responsibilities

• Establish relationships with patients, families and care team to coordinate care.
• Develop and communicate care plans; prioritize based on needs and preferences.
• Provide patient teaching and maintain accurate medical records.
• Contribute to unit performance and lead by example on service standards.
