Job summary

Responsibilities

• Plan, establish, and implement policies and procedures; develop individual or group goals and objectives and monitor, operating, evaluating, coordinating and overseeing staff of the CNH program.
• Work with Community Health Nurses and Community Care leadership to ensure adherence to directives, standards, and VA-OIG expectations; conduct ongoing reviews to meet objectives.
• Provide technical assistance to state, local, and private agencies and universities; monitor activities and program reviews and develop analysis of information.
• Develop crisis management plans and rapid assessments with the interdisciplinary treatment team; follow suicide risk assessment protocols when needed.
• Participate in meetings (IDT, team huddles, committees) and contribute to clinical reminders and performance measures.
