There is a lot of content here related to government reporting, equal employment opportunity statements, and disability disclosures. The form and associated text have been retained as part of the original content, but some of it has been condensed for readability in this reformatted version. This section includes sections such as demographic questions, veteran status, disability status, and related legal notices. See the original posting for full details.
Why are you being asked to complete this form?
We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.
Disability Status: Information about disabilities and related categories is provided for reporting purposes. This content is intended to be informative and compliant with applicable laws.
PUBLIC BURDEN STATEMENT: This survey should take about 5 minutes to complete.
