Submittd Talent is seeking a Founding Principal Engineer for a Series B healthtech startup focused on paediatric mental health. This hands-on IC role reports directly to the CEO and defines the technical foundation across telehealth, EHR, and AI.

You will own end-to-end systems, set coding standards with offshore teams, and decide what to build or buy. Requires 6–15 years as a staff or principal engineer, startup founder experience, and production TS stack (TypeScript/React/Node.js) on AWS.

Founding Principal Engineer, Series B HealthTech Start-Up, Paediatric Mental Health (Full-Stack)

New York, NY (Hybrid, 3 days in office)

I am working with a Series B health tech company that is rebuilding paediatric mental health care from the ground up. One in five children in the US has a diagnosable mental health condition, and around half never get care. The company runs a modern clinic for children and families that combines a digital platform, in-person clinics and telehealth. It is already live with patients and has recently raised its Series B.

This is their first onshore engineering hire. You'll work directly alongside the CEO and set the technical foundation for the company. This is a hands-on IC role, not a management one.

The Role

You'll write production code on the hardest problems and own the architecture across telehealth, the EHR, the patient dashboard and AI adoption. There are no detailed PRDs. You'll turn clinical, growth and operations needs into technical plans and make product decisions yourself.

Typical work includes:

• Building and owning systems end to end, from requirements through to UI
• Working directly with the CEO to scope, prioritise and ship product
• Working with the existing offshore engineering team and setting standards for code review and quality
• Driving AI adoption across the product and the wider organisation
• Making clear calls on what to build, what to buy and what not to build at all

TypeScript, React, Node.js, AWS, with AI and LLM integrations.

What they're looking for

• 6 to 15 years of full-stack engineering experience, currently working at Staff or Principal level
• Recent experience as a founding or very early engineer at a venture-backed start-up, ideally within the first five engineers
• Production full-stack TypeScript, with ownership across frontend and backend
• Strong product instincts, with a track record of shipping features that made a real difference for users
• Experience in a regulated environment such as healthcare, HIPAA or fintech
• A real interest in healthcare and the problem the company is solving
• Experience working with offshore or distributed teams and hands-on AI work in production are both a plus

The Opportunity

• Be the first in-house engineer and define how engineering is built from day one
• Work side by side with the CEO on a mission-driven product with real patients
• $200,000 to $250,000 base plus equity
• Hybrid, with 3 days a week in the office in New York City
• Open to OPT or H1B transfers, but unable to offer new visa sponsorship
