Role Overview

Join our Virtual Aged Care Services (VACS) team as a Social Worker. The role involves conducting psychosocial assessments, providing counselling, facilitating family and provider meetings, and contributing to case management within a multidisciplinary team. Many services are delivered in patients' homes and aged care facilities.

Candidate Requirements
  • Accredited Social Work Degree (Australian Association of Social Workers)
  • Experience in Aged Care, psychosocial stressors, and mandatory reporting
  • Skills in crisis intervention, grief counselling, conflict management
  • Ability to work independently, make clinical decisions, and apply professional judgement
  • Experience with NCAT applications
  • Commitment to professional development and supervision
  • Teamwork, computer skills, time management, communication skills
  • Current NSW driver's license and willingness to drive for work
