Responsibilities

• Develop assessment in collaboration with the interdisciplinary treatment team, Veteran, and family.
• Provide individual, group, and family psychotherapy and advanced case management.
• Collaborate with Veterans Administration and community resources for referrals and services.
• Create and update treatment plans with the Veteran and family, with goals.
• Provide crisis intervention and support as needed.
• Coordinate with primary care staff to address psychosocial needs and care plans.
