The Bristol Community Based Outpatient Clinic (Bristol CBOC) seeks a Senior Social Worker to provide evidence-based treatment and intensive case management for Veterans at an advanced practice level, promoting stability and addressing complex psychosocial needs.

Responsibilities

Interact with patient, family, community agencies and staff while providing clinical services to Veterans enrolled at Bristol CBOC; deliver high-quality case management, clinical treatment, advocacy, and coordination with VA or community providers.
Collaborate with the interdisciplinary treatment team; provide interventions through groups, individual psychotherapy, and family counseling.
Operate independently with little supervision; develop new methods and policies as needed.
Gather information to guide practice; provide specialized evidence-based treatment for complex physical/mental illness and a full range of social work services.
Offer services to homeless Veterans and ensure linkage to VA/community services; develop comprehensive treatment plans with the Veteran.
Teach and mentor staff, participate in policy-making, and collect data to improve performance and delivery of services.
