• Must have a master's degree in social work from a CSWE-accredited program.
• Must be licensed or certified as a social worker in a state.
• Proficiency in spoken and written English.
• U.S. citizenship required; English language proficiency as specified by VA policy.
• Experience in social work within clinical settings preferred.

Responsabilidades

• PC-MHI clinical practice incl. functional assessments and triage to specialty care.
• Collaborate with PCPs and PACT teams to plan care.
• Documentation within 24 hours with clear impressions and plans.
• Provide staff training and contribute to program development.
• Participate in program evaluation and clinical policy development.
