Job summary

NYC Health + Hospitals Woodhull in Brooklyn seeks a licensed LMSW or LCSW to join the OP BEHAV HLTH CHILD/ADOL department. You will perform psychosocial assessments, provide individual and group counseling, and coordinate community referrals to enhance patient outcomes.

Under supervision, you will support discharge planning, collaborate with interdisciplinary teams, and ensure compliance with NYC Health + Hospitals policies and state regulations while documenting patient care and progress.

Qualifications

• Valid license as an LMSW issued by the NYSED; or
• Valid license as an LCSW issued by the NYSED.
• Knowledge of psychosocial care and compliance with regulations.

Responsibilities

• Screens charts for historical and clinical information and conducts psychosocial evaluations.
• Provides individual and group counseling and concrete social services.
• Confers with professionals to develop multidisciplinary treatment plans.
• Prepares discharge and aftercare plans and facilitates referrals.
