Job summary

Qualifications

• Master's degree in social work from accredited university.
• Three years of healthcare social services experience.
• LCSW license in Texas required.
• Strong communication and discharge planning skills.

Responsibilities

• Role models effective communication with the healthcare team and patients/families; supports discharge planning and transitions.
• Provides staff education and mentorship to new employees.
• Leads psychosocial assessment, diagnosis, and treatment planning across units.
• Collaborates with physicians and interprofessional team to address psychosocial needs and improve outcomes.
