Job summary

The Veterans Health Administration seeks a Social Worker (Program Coordinator) for the HUD-VASH component of HCHV. This role provides direct case management and supports Veterans with serious mental illness within homeless programs, facilitating housing stability.

You will collaborate with VA providers, train staff, conduct intake and assessments, develop comprehensive treatment plans, and participate in performance improvement measures to improve outcomes for homeless Veterans.

Qualifications

  • Direct case management experience with Veterans.
  • Experience with HUD-VASH or homeless programs preferred.
  • Ability to develop professional relationships with Veterans and healthcare providers.
  • Ability to collaborate across multidisciplinary teams.
  • Experience in intake, assessment, and treatment planning.

Responsibilities

  • Provide SMI-focused services for the HUD-VASH program.
  • Engage Veterans and ensure continuity of care across Homeless Programs.
  • Develop comprehensive treatment plans addressing homelessness, SMI, SUD and co‑occurring factors.
  • Train and supervise social work trainees and unlicensed staff where appropriate.
  • Coordinate with VA providers and community partners; participate in performance improvement efforts.
