Job summary

Sanford Health Bemidji Medical Center is seeking a Dialysis Social Worker to support patients' social, emotional and economic needs as part of an interdisciplinary team. You will collaborate with clinical staff to develop effective treatment plans and provide brief therapy when appropriate.

The role requires a Master's in Social Work, MN licensure (LGSW or LICSW), and hospital or healthcare experience. This is a full-time position based in Bemidji, MN with day shifts and a competitive benefits

Qualifications

• Master's degree in Social Work from an accredited curriculum required.
• Healthcare and/or mental health hospital experience preferred.
• Must possess a license in good standing in state(s) of practice: In Minnesota, must have LGSW or LICSW.

Responsibilities

• Respond to suspected abuse, neglect or violence per NASW Ethics and state laws.
• Provide social, emotional and economic support within an interdisciplinary team.
• Apply age-appropriate care protocols across neonatal to geriatric patients.
• May deliver brief therapy and assist with treatment planning and care decisions.

The Dialysis Social Worker plays a vital role in supporting patients' emotional, mental health, and social needs while collaborating with the interdisciplinary team to develop effective treatment plans. Job Summary

Displays ability to apply the knowledge and skills of advanced social work practice, responding to suspected abuse, neglect or violence in accordance with the National Association of Social Work Code of Ethics policies and procedures alongside the appropriate state laws. Possesses an expert level of critical thinking and in-depth psychosocial assessments. Exhibits ability to apply complex concepts to situations. If carrying the correct licensure, may diagnose patients/clients/residents. May provide specialized counseling and support.

Serves as a member of the interdisciplinary team in providing assistance with social, emotional and economic concerns of the patients/clients/residents and families/caregivers, thus enabling them to achieve or maintain an optimal level of functioning. Utilizes appropriate age-related patient/client/resident care protocols relating to the physical and psychological needs of neonatal, pediatric, adolescent, adult or geriatric patient/client/resident. May work closely with the additional clinical staff to achieve fiscally sound and timely continued planning outcomes. Possesses advanced knowledge of the principles of teamwork. Understands the impact of illness on the patient/client/resident and families/caregivers. Demonstrates knowledge and clinical experience in the psychosocial and economic management of problems commonly occurring with an illness and may provide brief therapy as appropriate. Possesses ability to work with growth and development needs of patient/client/resident populations.

Qualifications

Master's degree in Social Work from an accredited curriculum required.

Healthcare and/or mental health hospital experience preferred.
