Job summary

Sanford Health Bemidji is seeking a Dialysis Social Worker to support patients’ emotional, mental health and social needs within an interdisciplinary team. You will apply advanced social work knowledge, participate in treatment planning, and help ensure compassionate, comprehensive care.

Responsibilities include addressing abuse/neglect per ethics and laws, providing counseling, and supporting patients and families to achieve optimal functioning across ages in a hospital setting.

Qualifications

• Master's degree in Social Work from an accredited curriculum required.
• Healthcare and/or mental health hospital experience preferred.
• Must possess a license in good standing in state(s) of practice: In Minnesota, must have one of the following: Licensed Graduate Social Worker (LGSW); Licensed Independent Clinical Social Worker (LICSW).

Responsibilities

• Respond to suspected abuse, neglect or violence in accordance with ethics and state laws.
• Provide specialized counseling and support as needed within the interdisciplinary team.
• Assist with social, emotional and economic concerns to help patients achieve optimal functioning.
• Apply age-appropriate care protocols across neonatal to geriatric populations.

Job Summary

Displays ability to apply the knowledge and skills of advanced social work practice, responding to suspected abuse, neglect or violence in accordance with the National Association of Social Work Code of Ethics policies and procedures alongside the appropriate state laws. Possesses an expert level of critical thinking and in-depth psychosocial assessments. Exhibits ability to apply complex concepts to situations. If carrying the correct licensure, may diagnose patients/clients/residents. May provide specialized counseling and support.

Serves as a member of the interdisciplinary team in providing assistance with social, emotional and economic concerns of the patients/clients/residents and families/caregivers, thus enabling them to achieve or maintain an optimal level of functioning. Utilizes appropriate age-related patient/client/resident care protocols relating to the physical and psychological needs of neonatal, pediatric, adolescent, adult or geriatric patient/client/resident. May work closely with the additional clinical staff to achieve fiscally sound and timely continued planning outcomes. Possesses advanced knowledge of the principles of teamwork. Understands the impact of illness on the patient/client/resident and families/caregivers. Demonstrates knowledge and clinical experience in the psychosocial and economic management of problems commonly occurring with an illness and may provide brief therapy as appropriate. Possesses ability to work with growth and development needs of patient/client/resident populations.

Qualifications
