Mecklenburg County’s Services For Adults (SFA) unit seeks a skilled social worker to provide community-based case management for adults, including assessment, planning, and referrals. This exempt position focuses on Adult Protective Services, Guardianship and in-home support within the Adult Social Work team.

Qualified candidates have a BSW (or MSW preferred) and at least three years of social work experience; a valid NC license is required, and occasional evenings/weekends may be needed.

Qualificações

• Bachelor of Social Work with three years of social work experience or equivalent
• MSW preferred; related experience accepted
• Requires valid North Carolina or South Carolina Driver's License and County Driving Privileges

Responsabilidades

• Assess needs of individuals and families
• Interview clients and collect social/medical information
• Determine eligibility for financial assistance and referrals
• Develop and evaluate service plans with clients and follow up on outcomes
• Refer clients to community resources and coordinate benefits and referrals
