Overview

This position is a remote Full-Time Day role (IUHMG-Metro).

The Virtual Integrated Behavioral Health team provides innovative services through a virtual model, collaborating with outpatient healthcare providers. Responsibilities include supporting patients' mental and physical health, coordinating with medical providers, screening and assessing for mental health and substance use disorders, facilitating engagement and follow-up care, tracking treatment response, and supporting psychotropic medication management. The team uses brief, evidence-based behavioral interventions and consults with psychiatric specialists for treatment updates. Members also develop relapse prevention plans for patients who have achieved their treatment goals.

Key responsibilities include:

  • Effectively communicating and collaborating with all healthcare team members to facilitate safe and timely patient care, providing necessary resources and education to patients and families.
  • Interpreting and communicating complex patient and family needs, role modeling caring practices.
  • Assessing psychosocial risk factors, evaluating prior functioning, support systems, reactions to illness, and coping abilities.
  • Counseling patients and families on emotional, social, and financial impacts of illness/disability, and mobilizing resources.
  • Providing mental health assessments upon physician referral.
  • Offering therapeutic interventions for discharge planning and crisis situations.
  • Serving as liaison with protective services and providing specialized mental health services, including crisis intervention and addiction assessments.
  • Supporting patients in crisis with de-escalation techniques and intervention.

Minimum qualifications include:

  • Master's degree in social work.
  • De-escalation certification within 6 months.
  • Permanent LSW licensure, LCSW preferred.
  • 3-5 years experience in relevant areas preferred.
  • Experience working with diverse populations.
  • Knowledge of disease impact, human behavior, legal mandates, growth and development principles, and crisis intervention techniques.
  • Proficiency in Microsoft Excel and Word.
