Ready to drive positive change in hospital social care? Join our Integrated Hospital Social Work Team as a Senior Social Work Practitioner and play a key role in improving outcomes for patients through safe, timely discharges and effective care planning.

Main duties of the job

As an experienced Social Worker, you will operate with a high degree of autonomy and authority, using a range of evidence-based social work interventions. You will manage complex cases and support the professional development of your team. You will collaborate closely with multidisciplinary teams, including our Complex Discharge and Access and Capacity Teams, to streamline transfer of care and discharge pathways, ensuring patients with complex needs are supported to return home with appropriate care in place.
