Brownsburg Health Care Center seeks an experienced Social Services Director to lead psychosocial services in our Skilled Nursing Facility (SNF). This role suits a licensed social worker passionate about resident advocacy, care planning, discharge planning, and interdisciplinary collaboration in a long-term care setting.

Qualifications

• Bachelor’s degree in Social Work or related Human Services field.
• Licensed, certified, or registered to practice in Indiana.
• Minimum 1 year of supervised social work experience in a healthcare setting.
• Knowledge of CMS regulations, resident rights, and discharge planning processes.

Responsibilities

• Complete comprehensive psychosocial assessments for residents.
• Develop and implement individualized care plans.
• Provide emotional, social, and mental health support to residents and families.
• Lead discharge planning, transitions of care, and community resource referrals.
• Advocate for resident rights, dignity, and quality of life.
• Collaborate with nursing, therapy, and interdisciplinary care teams.
• Maintain accurate documentation in compliance with state and federal regulations.
• Participate in state surveys, audits, and compliance reviews.
• Supervise or support social services staff, as applicable.
