Job summary

Qualifications

• Current, full, active, and unrestricted RN license in the United States.
• Graduate of a school of professional nursing approved by accrediting bodies (ACEN/CCNE).
• State licensure or pathway to licensure via bridge program if applicable.

Responsibilities

• Provide comprehensive case management across inpatient, outpatient, and home-based Veterans care settings.
• Coordinate and oversee Home Healthcare services for eligible Veterans.
• Develop, implement, and monitor individualized care plans.
• Facilitate transitions and ensure timely access to services and appropriate follow-up.
