The role requires an active RN license in the United States, ACEN/CCNE-accredited nursing education, and licensure through a bridge pathway if applicable. Strong collaboration with Veterans, families, and the healthcare team is essential for safe transitions and high-quality care.

Qualifications

• English language proficiency required.
• Graduate of an ACEN/CCNE accredited nursing program.
• Current unrestricted RN license in the United States.
• Bridge program pathway available for licensure prior to completion of bridge.
• Foreign-educated nurses must obtain degree equivalency evaluation.

Responsibilities

• Provide comprehensive case management and care coordination for Veterans across inpatient, outpatient, and home-based settings.
• Coordinate Home Healthcare services for eligible Veterans and ensure follow-up with providers.
• Conduct outpatient Annual Examinations and ensure evaluations are completed.
• Develop and monitor individualized care plans and document in EHR.
• Collaborate with Veterans, families, and interprofessional teams to ensure continuity of care.
• Identify barriers to care and assist in developing solutions to improve access and transitions.
