Job summary

The Alliance in Central California is seeking a Senior Complex Case Manager (RN) in a hybrid role to lead D-SNP case management efforts. You will develop individualized care plans and coordinate across providers, aging and chronic care needs, while supporting cost-effective, quality outcomes for Medicare members.

Ideal candidates hold a CA RN license with seven years of patient care experience and at least one year in case management; a Bachelor’s degree can substitute for two years of

Qualifications

• Current unrestricted Registered Nurse license issued by California.
• Associates degree in Nursing; Bachelor’s may substitute for two years of experience.
• Minimum of seven years patient care experience with acute or outpatient setting.
• At least one year in case management; ability to work with dual-eligible populations.

Responsibilities

• Develops and manages individualized care plans for D-SNP members.
• Educates members, families, providers, and internal teams on the D-SNP program.
• Participates in Quality Improvement studies to enhance outcomes and cost effectiveness.
• Oversees D-SNP program activities and supports implementation with providers.
• Acts as liaison between D-SNP program and community agencies.
