Job summary

Central California Alliance for Health is seeking a Senior Complex Case Manager (RN) in the Care Management Department. The role focuses on developing comprehensive care plans for Medicare D-SNP members, coordinating with providers, and guiding care delivery across teams.

Candidates should have CA RN licensure and substantial case management experience, with a strong emphasis on geriatric and dual-eligible populations.

Qualifications

• Current unrestricted license as a Registered Nurse in California.
• Associate's degree in Nursing with at least seven years of patient care, including one year in case management.
• One year of patient care in acute or outpatient settings, with adults or dual-eligible populations.

Responsibilities

• Develops and manages an individualized care plan for Medicare D-SNP members in Case Management.
• Educates members, families, providers, external agencies, and internal departments on the D-SNP Case Management Program.
• Participates in Quality Improvement studies to evaluate program effectiveness and outcomes.
• Performs D-SNP Program oversight and support activities.
• Liaises between the D-SNP Program and providers/community agencies to implement program objectives.
