Job summary

You will guide patients through UCLA Health's continuum of care, coordinating transfers and acting as the primary contact for international patients while collaborating with diverse medical teams.

Qualifications

• BSN degree and California State RN license.
• Ability to complete a Nursing physical assessment.
• Develop a plan of care with measurable goals.
• Implement interventions aligned with the plan of care.
• Evaluate interventions and reassess as needed.
• Educate patients, families and referring physicians.
• Triage inquiries and referrals with quick turnaround.
• Communicate effectively in writing and verbally.
• Navigate multiple UCLA Health specialties and coordinate transfers.

Responsibilities
