Responsibilities

• Assess and document clinical, psychosocial, and financial needs of patients.
• Apply InterQual to determine/validate Level of Service and Intensity of Care.
• Coordinate with physicians to create transition/discharge plans.
• Monitor treatment plans and prepare for patient transitions.
• Ensure timely patient discharges and document variances.
• Escalate concerns regarding treatment or transitions.
• Maintain knowledge of facilities and resources available to patients.
