Qualifications

Responsibilities
Assess and document clinical, psychosocial, and financial needs of patients.
Apply InterQual to determine/validate Level of Service and Intensity of Care.
Coordinate with physicians to create transition/discharge plans.
Monitor treatment plans and prepare for patient transitions.
Ensure timely patient discharges and document variances.
Escalate concerns regarding treatment or transitions.
Maintain knowledge of facilities and resources available to patients.
