Reporting to the Regional Director of Care Management, you will support the Care Management team in coordinating care and performing complex tasks in patient assessment and management.

Qualifications

• 5 years hospital/clinical or healthcare field experience.
• 3 years outstanding customer service experience.
• 3 years clerical and computer experience.
• 3 years knowledge of insurance billing and managed care.

Responsibilities

• Provide support to the Care Management team.
• Assist in coordinating patient assessments and care planning.
• Collaborate with healthcare team to carry out care management activities.
• Perform complex tasks identified during initial and ongoing assessment and management of patients.
