Job Description

Housing for Health (HfH) at NYC Health + Hospitals (H+H) recognizes that the chronic health issues of unhoused individuals cannot be treated without stable housing. This initiative seeks to improve the health and wellbeing of New Yorkers by focusing on four strategic areas: housing navigation services, medical respite beds, affordable housing on hospital property, and social service support for patients in permanent housing.

The Housing team works closely with H+H care teams to identify patients; accesses suitable opportunities through strategic partnerships with government agencies, housing developers and nonprofit organizations; and supports patients through their progression to permanent housing. The team reports to the Senior Assistant Vice President of Housing and Real Estate and sits within the Office of Managed Care. This Project Manager (Grant Funded) role will report to the Assistant Director for Housing Policy and Operations.

Housing for Health is launching a new initiative to streamline the application and placement process into supportive housing for H+H patients. Recognizing the obstacles that many H+H patients face in accessing supportive housing, Housing for Health intends to optimize the supportive housing application process by implementing the following strategies:

  • Training care team members across NYC Health + Hospitals and at contracted vendors to complete supportive housing applications in CAPS (NYC’s coordinated entry system);
  • Granting patients access to expedited Mental Health Reports through Virtual ExpressCare, H+H’s telehealth platform, to align with the clinical documentation required to receive supportive housing approvals.
  • Working with the Department of Social Services and other city and state government agencies to improve collaboration and the supportive housing referral and placement process;
  • Data and technology enhancements to application systems; and
  • Referrals to Housing for Health’s housing location and placement services team for support in the placement, lease-up, and transition process.

The Senior Project Manager (Grant Funded) will be responsible for supporting Housing for Health to grow its supportive housing strategy and facilitate permanent housing placements for patients experiencing homelessness.

General tasks and responsibilities include but not limited to:

  • Provide training and technical assistance on the supportive housing workflow to H+H facility partners care teams, and H+H vendors.
  • Support H+H housing navigation and respite vendors to complete supportive housing applications.
  • Perform patient intake for housing navigation, including reviewing H+H facility referrals to Housing for Health to confirm patient eligibility; liaising with referral sources for additional patient information and consent; and matching patients to a housing navigation vendor.
  • Manage the Mental Health Report referral process for supportive housing applications, including document collection, chart review, and clinical summary, in partnership with Virtual ExpressCare, H+H facility partners, medical respite vendors, and the housing navigation team.
  • Support referral and placement opportunities for H+H patients into housing,
  • Support day to day data management, including analysis and reporting to monitor program performance, goals, and quality improvement efforts; matching patients to housing opportunities; identification of patient housing barriers; and data clean up.
  • Merge and parse data supplied by all Housing for Health programs, including through Epic, CAPS, and internal Housing for Health datasets, and be comfortable transforming dirty big data into actionable knowledge.
  • Communicate analyses clearly to lay audiences and use analysis to drive progress towards H+H housing goals with facility partners, external entities, and vendors.
  • Assists in designing evaluative measures including data analysis and procedures by which program efficiency and cost effectiveness can be audited and assist in evaluation audits.
  • Program and project management for other housing, medical respite, and real estate initiatives as assigned.
  • Support Housing for Health’s needs across all programs as requested.

Minimum Qualifications
  • Baccalaureate degree from an accredited college or university in Business or Public Administration, Healthcare Management, Engineering, Social Sciences, Physical Sciences or related discipline; and
  • Six (6) years of progressively responsible experience in project and program management, execution, and implementation, three (3) years of which must have been in the administration, management and/or execution of projects and programs of various complexities, including work in health care program planning, research, design or related area; or
  • Certification in Project Management (e.g., Project Management Professional (PMP) certification, Program Management Professional (PgMP), Certified Associate in Project Management (CAPM)) and four (4) years of experience as described in #2 above, two (2) years of which must have been in the administration, management and/or execution of projects and programs of various complexities, including work in health care program planning, research, design or related area.

Preference will be given to qualified candidates with the following knowledge, abilities, education, experience and/or skills:

Educational/Experience Level:

  • Baccalaureate degree from an accredited college or university in Business or Public Administration, Healthcare Management, Engineering, Social Sciences, Physical Sciences or related discipline; and
  • Graduate degree in public administration, social work, public health, or similar discipline OR
  • Undergraduate degree with at least five years’ experience with project management and hands on data analysis OR
  • A satisfactory equivalent of education, training and/or experience

Knowledge, Skills, Abilities and other Requirements:

  • New York City residency (Brooklyn, Bronx, Manhattan, Queens or Staten Island) preferred.
  • Interest in housing and homelessness in a healthcare setting
  • Experience with project management
  • Excellent written and oral communication skills.
