Form Health is a fully remote, physician-led obesity and cardiometabolic care platform. The Care Delivery team builds tools that power prescribing, prior authorizations, and virtual visit workflows for a high-performing virtual practice.

We seek a PM who thrives on complex, regulated problems, with experience shipping AI-powered features and a track record of coordinating across clinical, payer, and operations teams.

Qualifications

• 5+ years building enterprise experiences with complex workflows.
• Comfortable diving into technical problems — APIs and workflows spanning homegrown and third-party systems.
• Clear communicator cross-functionally in a remote-first environment.
• Fluent in using AI as part of your PM craft — drafting specs and prototyping.

Responsibilities

• Own care-delivery capabilities end-to-end — e.g., prescribing, prior authorizations, virtual visit workflows.
• Translate business and user needs into clear product requirements, including user stories and specs.
• Partner with engineering, design, operations, and clinical teams to ship tools for our clinical practice.
• Track program metrics and report progress and health to Form Health's leadership.
• Facilitate sprint planning, standups, backlog grooming, and retrospectives.
• Identify where AI can reduce clinical or administrative burden and partner with engineering to ship those capabilities thoughtfully.

Form Health is a virtual obesity medicine clinic delivering physician-led, evidence-based care for obesity and related cardiometabolic conditions. Obesity affects more than 40% of U.S. adults, and as GLP-1 medications reshape the field, Obesity and Cardiometabolic Medicine is in a period of rapid growth and opportunity. Every Form Health patient works closely with a dedicated multidisciplinary Care Team, including board-certified physicians (many ABOM-certified, and endocrinologists), advanced practice providers, and Registered Dietitians who together deliver comprehensive, one-on-one treatment that includes nutrition, behavioral health, physical activity, and FDA-approved medication, if appropriate. Through our mobile app, patients engage in regular video visits along with text messaging, photo journaling, and personalized educational content. About the Role:

The Care Delivery team builds the tools that let our clinical team run a high-performing virtual practice — from prescribing and prior authorizations to the day-to-day workflows clinicians rely on to do their best work with patients. This role is highly visible within Form Health: the tools you ship directly shape how our clinical team practices and, downstream, how our patients experience care. We're looking for someone who's genuinely energized by hard, regulated, multi-stakeholder problems — with clinical, patient, payer, and operational constraints — as well as someone who uses AI as a real part of supercharging their work, rather than a buzzword on a resume.

This role will report to our Lead Product Manager for Care Delivery, with significant ownership from day one and plenty of room to help shape how the practice scales as the team grows. This role is fully remote and can be located anywhere in the U.S. (Note: while not required, it is beneficial to be in or close to the Eastern Time zone given frequent collaboration with our Boston-based clinical and operations leaders)

What You Will Do:

• Deeply understand Form Health's clinical and business needs: identify and prioritize jobs-to-be-done using research and user feedback
• Own key care delivery capabilities end-to-end — e.g., prescribing, prior authorizations, virtual visit workflows
• Partner with clinical leadership to triage and prioritize incoming product requests, balancing clinical needs against broader roadmap priorities
• Translate business and user needs into clear product requirements, including user stories, use cases, and functional specs
• Partner with engineering, design, operations, and clinical teams to ship best-in-class tools for our clinical practice
• Track key program and product metrics for your area, and report on progress and health to Form Health's leadership team
• Facilitate sprint planning, standups, backlog grooming, and retrospectives to keep the team aligned and shipping
• Partner with clinical education and enablement to roll out new features smoothly — change management, communications, and training
• Identify where AI and automation can responsibly reduce clinical or administrative burden (e.g., documentation, prior-auth workflows, care coordination), and partner with engineering to evaluate and ship those capabilities thoughtfully

About You:

• 5+ years building enterprise experiences with complex workflows
• Comfortable diving into technical problems — APIs, workflows spanning homegrown and third-party systems
• Clear communicator cross-functionally in a remote-first environment
• A track record that includes:
• Evaluating third-party vendors and driving coordination across multiple systems
• Defining where a product should go, rallying a team behind it, and delivering results iteratively
• Owning a product area end-to-end — strategy and prioritization through rollout and adoption
• Fluent in using AI as part of your own PM craft — drafting specs, synthesizing research, prototyping — and curious about where AI can responsibly augment clinical operations
• Proactive ownership over your area of responsibility, with a strong sense of accountability and an execution-oriented mindset — comfortable applying structure to ambiguous situations in a fast-growing startup
• Genuinely excited to make compassionate, evidence-based healthcare accessible — and energized, not deterred, by the complexity of a regulated, high-stakes domain

Preferred:

• Understanding of healthcare, virtual care delivery, or health insurance
• Experience shipping AI-powered features, ideally within a tech-enabled services organization
• Prior experience in a startup and/or high growth environment
