Job summary

Humana's CenterWell Senior Primary Care in South Carolina is seeking a physician to provide in-visit and ongoing patient management in a full-time, on-site role. You will report to the Associate Medical Director and be responsible for both in-visit care and ongoing patient management.

You will see 12-15 patients per day, manage patients with complex and chronic conditions, and complete documentation to support patient care. A SC license and board eligibility are required.

Qualifications

• MD or DO from an accredited program.
• Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics.
• Active, unrestricted medical license in South Carolina.

Responsibilities

• See 12-15 patients per day with longer, in-depth visits.
• Manage patients with complex and chronic conditions.
• Complete accurate, same-day documentation to support care.
• Manage a daily clinical inbox (labs, messages, follow-ups).
• Coordinate care with specialists and care team members.
• Apply Medicare risk adjustment principles through documentation.
