Job summary

CenterWell Senior Primary Care, a Humana subsidiary, seeks a Primary Care Physician (MD/DO) for an outpatient clinic in Columbia, South Carolina. This is a full-time, onsite, patient-facing role reporting to the Associate Medical Director.

You will provide comprehensive primary care services to adults and seniors, manage chronic conditions, see 12-15 patients per day with longer visits, and coordinate care within a value-based care model.

Qualifications

• MD or DO from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Active, unrestricted medical license in South Carolina or ability to obtain licensure.
• Active, unrestricted DEA registration or ability to obtain prior to start date.

Responsibilities

• Provide comprehensive primary care services to a panel of adult and senior patients.
• Care for patients with chronic and complex medical conditions.
• Typically see 12-15 patients per day, allowing for longer visits and stronger patient relationships.
• Manage longitudinal care plans focused on prevention, wellness, and chronic disease management.
• Complete accurate, same-day documentation to support quality outcomes and patient care.
• Manage a daily clinical inbox, including lab review, patient messages, prescriptions, and follow-up care.
• Coordinate care with specialists, hospitals, community resources, and members of the care team.
• Participate in quality improvement initiatives and support value-based care goals.
• Apply Medicare risk adjustment and annual condition documentation practices to support comprehensive patient care.
