Responsibilities:
  • Conduct comprehensive assessments, including physical, emotional, and mental status evaluations for clients of all ages—from infants to adults.
  • Develop care plans with realistic outcomes based on assessment data and client preferences.
  • Perform designated discipline interventions according to agency policies.
  • Provide client and family education on illness prevention, management, medications, and treatments.
  • Evaluate clinical intervention outcomes to measure progress toward goals.
  • Report pertinent client status and progress information to the multidisciplinary health team.
  • Plan home visit sequences considering acuity, geographical location, and other relevant factors.
Requirements:
  • Graduate of an American Physical Therapy Association-approved Physical Therapy program.
  • Two years of senior-level experience in the field or in an acute medical hospital setting.
  • Supervised clinical experience in a medical setting preferred, or an equivalent combination of education and experience.
  • Valid driver’s license and ability to transport oneself for home visits.

Hours: M-F 8:30 am – 5:00 pm, with some weekends and holidays.
