Job summary
The Hillingdon Hospitals NHS Foundation Trust is seeking a Senior Pharmacist Team Leader for Care of the Elderly and Rehabilitation to lead clinical pharmacy services across inpatient care and drive medicines optimisation within elderly care, frailty and rehabilitation.
You will provide professional and managerial leadership to the pharmacy team, ensure safe and timely medication supply, contribute to policy development and support discharge planning and governance within the Directorate.
Responsibilities
- Ensure compliance with medicines legislation and internal policies.
- Ensure safe, legal and timely supply of medication to patients.
- Lead day-to-day planning and provision of clinical pharmacy services.
- Promote cost-effective prescribing and medicines optimisation.
- Develop clinical guidelines, PGDs and policies.
- Provide professional and managerial support to senior pharmacists.
- Lead the pharmacy team and discharge medicines information transfer.
