Job summary

Community Health Network in Indiana is seeking a Pharmacist to join the Specialty Pharmacy team, providing clinical and operational support for ambulatory clinics and performing inpatient order set verification.

The ideal candidate has a PharmD, a valid Indiana license, and 4–5 years of experience in specialty or hospital-based pharmacy, including benefits investigation, patient education, and monitoring laboratory results and pharmacogenomics where applicable.

Qualifications

• Requires PharmD and active Indiana pharmacist license.
• Experience in specialty, ambulatory, hospital-based, or closed-door long-term care pharmacy.
• Ability to create and maintain clinical pathways for specialty therapies, including monitoring of labs, pharmacokinetics, and pharmacogenomics.

Responsibilities

• Provide clinical and operational support for ambulatory clinics in the Specialty Pharmacy service line.
• Perform inpatient order set verification.
• Develop and maintain clinical pharmacy pathways for initiation and monitoring of specialty medication therapy.
• Educate patients and monitor pharmacotherapy outcomes.
