You will coordinate care, intervene crisis situations, advocate for patients and families, and connect them to resources to improve outcomes.

The role emphasizes collaboration with a diverse multidisciplinary team to address the needs of children and young adults dealing with chronic health issues.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Current license to practice social work in Michigan (LMSW or LLMSW).
• Two years' experience with families and children around chronic illness, adjustment to illness and disability, school issues, and resources.

Responsibilities

• Evaluate psychosocial needs and provide billable social work interventions with autonomy.
• Advocate for patients and families navigating healthcare systems.
• Counsel patients and families on coping with chronic illness.
• Collaborate with multidisciplinary teams on care plans.
• Coordinate with Child Protection and community agencies to support chronically ill children.
• Participate in Social Work meetings and committees.
