Qualifications

• Master's Degree in Social Work required.
• Licensed Independent Clinical Social Worker (LICSW) required.
• 3–5 years of clinical social work experience.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition and treatment plans.
• Provides mandated assessments when abuse is suspected and specialized safety assessment when violence is suspected.
• Collaborate with other professionals to evaluate patients' medical or physical condition and assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
• Screen, address, and manage social determinants of health.
• Prepares and maintains all pertinent records, statistics, and progress notes.
