Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT with active state license.
• Experience delivering care in skilled nursing or post-acute settings preferred.
• Ability to work onsite and document patient progress.
• Strong communication with families and interdisciplinary teams.

Responsibilities

• Evaluate residents and determine treatment goals.
• Develop individualized, evidence-based treatment plans.
• Deliver interventions to improve ADLs, mobility, cognition, and safety.
• Document patient progress in EMR and maintain accurate records.
• Collaborate with interdisciplinary team to optimize resident outcomes.
