Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT in good standing
• Experience in skilled nursing or post-acute settings preferred
• Onsite work availability required

Responsibilities

• Evaluate residents and determine OT needs
• Develop individualized treatment plans and goals
• Deliver evidence-based interventions for ADLs, mobility, cognition, and safety
