Job summary

West Liberty Nursing & Rehab is seeking a full-time Occupational Therapist to evaluate and treat residents, focusing on ADLs, functional mobility, and independent living. You will work with an interdisciplinary team to deliver resident-centered care in a geriatric setting.

Responsibilities include developing and adjusting treatment plans, educating residents and families, supervising COTAs, and ensuring documentation and compliance with policies.

Qualifications

• Master’s or Doctorate in Occupational Therapy from an accredited program.
• Current state licensure as an Occupational Therapist.
• Minimum 2 years of experience in occupational therapy, preferably in geriatric or long-term care.

Responsibilities

• Conduct comprehensive evaluations to assess residents’ physical, cognitive, and psychosocial function.
• Develop individualized treatment plans focused on ADLs, self-care, mobility, and safety.
• Implement OT interventions including therapeutic exercises, ADL training, adaptive equipment use, and task-specific activities.
• Monitor and document residents’ progress and adjust plans as needed.
• Educate residents, families, and caregivers on plans, home exercises, and safety strategies.
• Collaborate with interdisciplinary team to ensure coordinated, resident-centered care.
• Supervise Certified Occupational Therapy Assistants (COTAs) per state and facility guidelines.

Education
