Job summary

Crossover Health is seeking a Nurse Practitioner to meet clinical, administrative, and financial performance metrics, delivering high-quality patient-centered care within our Primary Health model. You will provide comprehensive primary care, collaborate on treatment plans, supervise staff, and be available for after-hours questions, with a multidisciplinary care team.

Board certification, active state license, 5+ years in Internal Medicine or Family Practice, and current BLS are required;

Qualifications

• Minimum of 5 years of clinical experience in Internal Medicine or Family Practice.
• Must be Board Certified.
• Must be licensed to practice in appropriate state.
• Current BLS (Basic Life Support) certification required.

Responsibilities

• Provide patient centered, primary care services.
• Participate in the selection of suitable treatment plans for all patients.
• Supervise staff to ensure the care team provides appropriate care.
• Assist other care providers and extended care teams.
• Provide continuous availability for medical and technical questions, including after hours coverage.
• Actively participate in quality improvement programs and multidisciplinary QA programs.
• Collaborate with management to improve patient care.
• Cooperate with clinical outcome reviews and measurements.
• Comply with all policies, procedures, and protocols.
• Provide general medical care under the direction of a physician (depending on state).
• Submit health care plan and goals for periodic review.
• Prescribe or recommend drugs or other therapies.
• Refer patients to physicians or specialists as needed.
• Perform other duties as assigned.
