Qualifications

• RN license in Texas or a compact state.
• Associate's Degree in Nursing.
• 3 to 5 years of healthcare experience in care coordination or case management.

Responsibilities

• Oversee development and management of an integrated medical benefit program including case management and health/disease management.
• Coordinate caseload patient care across the continuum.
• Interact with providers and plan members in case management activities to provide efficient service.
• Identify high-risk individuals and develop programs to reduce financial and health risks.
• Monitor and evaluate the effects of case management on the patient population.
• Prepare cost analyses and reports to determine reasonableness of claims.
