Job summary

Pacific Clinics is seeking a Mental Health Worker II to deliver direct support for adults with behavioral health needs across outpatient, residential, or community settings. You will help implement individualized service plans, document progress, and may lead fellow staff under clinical supervision.

The role emphasizes client-centered care, crisis support, and collaboration with clinical teams to promote recovery goals and safety.

Qualifications

• High School Diploma or GED plus 4+ years of relevant experience.
• Three years experience with adults experiencing mental illness, trauma, or co-occurring conditions.
• Familiarity with behavioral health documentation and Medi-Cal standards.
• Experience in residential, outpatient, or field-based mental health programs.
• CPR/First Aid and crisis de-escalation certification (or willingness to obtain).

Responsibilities

• Provides direct support to clients through therapeutic activities and skill-building.
• Documents client progress toward goals and implements treatment plans.
• Participates in intakes, assessments, and case planning.
• Conducts wellness calls and crisis support under clinical supervision.
• Supports clients in navigating resources and appointments.
• Documents services in electronic health records per guidelines.
• Maintains a safe, inclusive environment across settings.
• Mentors newer staff or trainees and participates in trainings.
