Qualifications

• Active Maryland license or certification required.
• Licensure options include LCPC/LGPC, LCSW variants, and other eligible credentials.
• Experience in outpatient mental health and care coordination is preferred.
• Strong documentation and treatment planning skills are essential.

Responsibilities

• Provide weekly clinical support to assigned clients.
• Coordinate assessments, treatment planning, and progress monitoring.
• Coordinate referrals with therapists, PRP staff, and community providers.
• Track referrals for services like housing, substance use treatment, and primary care.
• Support engagement, care transitions, and continuity of care.
• Maintain accurate EMR documentation and ensure compliance.
