As Medical Social Worker, you will shape care delivery, mentor a team of social workers, and help set the standard for high-quality service. If you love the clinical side of social work and are ready to take on leadership or grow your leadership skills, we'd love to meet you.

Qualifications

• MSW from an accredited program.
• Active California LCSW license.
• At least 3 years of management experience, preferably in home care.
• Knowledge of healthcare regulations and ACHC standards.
• Strong communication and trust-building with patients, families, and staff.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers - providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build strong knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team's skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.
