WHY CARE SYNERGY?

Care Synergy is a Colorado-based nonprofit healthcare organization providing shared services and specialized expertise to Affiliates delivering compassionate care throughout the Front Range.

Our Affiliates serve individuals and families through hospice, palliative care, PACE, and other community-based programs. Behind that care is a dedicated team of professionals working together to strengthen quality, compliance, and operational effectiveness.

At Care Synergy, your expertise matters. You'll be part of a collaborative organization where your work contributes to the financial sustainability and quality of programs that make a meaningful difference in our communities.

THE DIFFERENCE YOU’LL MAKE

As our Enterprise Medical Coding Specialist, you'll play an important role in ensuring clinical documentation is accurately translated into appropriate medical codes, supporting regulatory compliance, reliable reporting, and appropriate reimbursement.

You'll work across three distinct areas of healthcare, partnering with providers, clinical leaders, Health Information Management (HIM), Compliance, and Revenue Cycle teams to improve coding accuracy and strengthen documentation practices.

WHAT YOU’LLDO:
  • Review clinical documentation and assign accurate ICD-10-CM diagnosis codes for Hospice, Palliative Care, and PACE services.
  • Ensure coding accurately reflects documented diagnoses and complies with CMS, payer, and applicable regulatory requirements.
  • Support hospice coding, including principal and additional diagnoses, admissions, recertifications, and diagnosis updates.
  • Review palliative care encounters and apply appropriate diagnosis and encounter coding, including CPT and HCPCS codes when applicable to assigned professional services.
  • Support PACE diagnosis coding, including Medicare risk adjustment and hierarchical condition category (HCC) requirements.
  • Partner with physicians, nurse practitioners, and clinical teams to clarify documentation and resolve coding discrepancies.
  • Identify documentation gaps, coding trends, and opportunities to improve accuracy and consistency.
  • Collaborate with Revenue Cycle to resolve coding-related claim edits, denials, and reimbursement concerns.
  • Participate in coding audits, quality reviews, and process improvement initiatives.
  • Serve as a trusted coding resource, helping educate clinical teams on documentation and coding best practices.
WHAT YOU'RE GOING TO LOVE:

Meaningful work. Your expertise supports programs that care for individuals with serious illnesses, older adults, and families throughout Colorado.

Variety in your day. You'll work across Hospice, Palliative Care, and PACE, applying your knowledge to different clinical specialties and coding requirements.

Collaboration that matters. You'll partner with experienced clinical, compliance, HIM, and revenue cycle professionals who value accuracy, integrity, and teamwork.

An opportunity to make an impact. You'll help strengthen enterprise coding practices, improve documentation quality, and contribute to processes that support sustainable, high-quality care.

A mission-driven environment. You'll be part of a nonprofit healthcare organization where the work behind the scenes is just as important to our mission as the care delivered directly to patients and families.

WHAT YOU’LL NEED TO SUCCEED:

Required Qualifications:

  • High school diploma or equivalent.
  • Current Certified Coding Specialist (CCS), Certified Professional Coder (CPC), or Certified Coding Specialist–Physician-based (CCS-P) credential.
  • Minimum three years of professional medical coding experience.
  • Strong knowledge of ICD-10-CM coding guidelines, medical terminology, and clinical documentation requirements.
  • Experience reviewing electronic medical records and assigning accurate diagnosis codes.
  • Understanding of Medicare coding and documentation requirements.
  • Strong analytical skills, attention to detail, and ability to manage competing priorities.
  • Effective communication skills and a collaborative approach to working with clinical and administrative teams.
Preferred Qualifications
  • Associate degree in Health Information Management, Health Information Technology, or a related field.
  • Experience with hospice, palliative care, PACE, or other community-based healthcare services.
  • Knowledge of Medicare risk adjustment and HCC coding.
  • Experience with professional services coding, including CPT and HCPCS.
  • Experience supporting coding audits, documentation improvement, or coding-related denial resolution.
  • Familiarity with electronic health record systems and coding software.
