As a Sr. Manager, you will play a critical role in supporting our Oak Street Health patients, providers and business by supporting the successful execution, adoption, and operationalization of care management operations and new care management initiatives across Oak Street Health centers. This role serves as a key bridge between centralized care management strategy and field operations, partnering closely with center and regional leaders to drive understanding, alignment, and adoption of evolving care management workflows, processes, and team structures. The ideal candidate is a highly collaborative operational leader with deep knowledge of interdisciplinary care management models and strong experience navigating complex operational and organizational change in a fast-paced healthcare environment. This role is focused on field enablement, operational execution, continuous improvement, and cross-functional partnership-not direct clinical precepting or direct people management of care management staff.

The typical pay range for this role is:
$82,940.00 - $182,549.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company's equity award program.
