Job summary

Medica is seeking a Manager of Case Management to lead daily operations for Health Services Case Management across Medica’s commercial and government lines. This role directs clinical staff, ensures NCQA compliance, and partners with provider teams to optimize care transitions and population health management.

The position requires RN and MSW qualifications, 8+ years of related experience, and an onsite work arrangement with potential Madison, WI or Minnetonka, MN offices.

Qualifications

• Associate's or Bachelor's degree in Nursing required.
• Master's degree in Social Work required.
• CCM certification preferred or ability to obtain within two years.

Responsibilities
