Job summary

Health & Human Services Commission (HHSC) in Plano, TX, is seeking an Investigator V to conduct on-site inspections and investigations in long-term care facilities to ensure compliance with state and federal rules. The role emphasizes report writing, regulatory determinations, and collaboration with facility staff.

Under supervision, you’ll perform licensure inspections, complaints and incidents investigations, and surveys across facilities statewide.

Qualifications

• Graduation from an accredited four-year college or university; investigative experience may substitute.
• Experience in long-term care preferred.
• Experience writing detailed reports.
• Ability to operate computers and software.

Responsibilities

• Conduct on-site licensure inspections and incident investigations to determine compliance with state and federal regulations.
• Perform annual comprehensive surveys in facilities and prepare detailed reports of deficiencies.
• Communicate clearly with internal and external stakeholders to resolve issues and answer inquiries.
• Conduct exit conferences with facility staff and verify correction of cited violations.
• Travel within a region and across the state; may be required for emergencies or hearings.
