Job summary

StoryPoint is seeking a Wellness Director to guide the overall direction, administration and coordination of clinical resident care services, delivering a 1440 experience and optimizing performance within our community.

You will lead a team, manage revenue and care plans, and coordinate Alzheimer’s/Dementia programs while upholding our care standards. This role requires nursing licensure and leadership capabilities, with a focus on resident well-being and stakeholder collaboration.

Qualifications

• Licensed nurse (LPN or RN) registered in the state preferred.
• CDP required prior to or upon hire within 90 days.
• Assisted Living/Memory Care experience preferred.

Responsibilities

• Lead and be accountable for your team and their performance.
• Manage revenue for Wellness, evaluations, care conferences and service plans.
• Coordinate Alzheimer’s and Dementia programs; ensure 1440 care standards.
• Communicate the organization’s vision to staff, residents, and partners.
• Develop relationships with community leaders and external partners.
