Job summary

Guardian Angels Senior Services in Elk River, MN is seeking a Speech-Language Pathologist to provide and oversee services for residents, evaluating, planning, and documenting therapy per physician orders, and supervising Rehabilitation Aides.

You will develop treatment plans, coordinate with the care team, counsel families, participate in discharge planning, and ensure compliance with state regulations while advancing clinical excellence.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited program.
• Certificate of Clinical Competency and Minnesota licensure.

Responsibilities

• Provides and oversees Speech-Language Pathology services for residents/clients, including evaluation, treatment, development and execution of therapy plans, and completion of all required documentation.
• Coordinates treatment plans with resident/client, family, and other team members; develops home or community programming.
• Supervises Rehabilitation Aides; participates in discharge planning; adheres to facility policies and state regulations.
