Job summary

Lifespace Communities in Dallas, Texas, invites a Social Worker to join our team at Querencia at Barton Creek in Austin. You will assess psycho-social needs, document care plans, and support move-ins to ensure residents receive compassionate, person-centered care.

The role focuses on resident advocacy, collaboration with families and clinical staff, and adherence to regulatory standards for advance directives and care planning.

Qualifications

• Master’s in social work required.
• Licensed Social Worker per state regulations.
• Experience in health care setting preferred.

Responsibilities

• Assess and document psycho-social needs and data related to resident illness and care.
• Attend care planning meetings and complete social service documentation (MDS and care plans).
• Assist in inquiry process, move-in coordination, and paperwork; conduct tours for new move-ins.
• Visit new residents to introduce self and verify insurance, and inform about non-covered costs.
• Provide a high level of resident engagement and advocacy.
• Coordinate with residents, families, team members, and outside agencies for in-house services.
• Support departments with resident rights training and compliance.
